Population health management en sistemas de salud fragmentados: hacia un modelo conceptual adaptado al contexto peruano para la coordinación intersectorial y la reducción de inequidades en salud

El sistema de salud peruano presenta una marcada fragmentación institucional que genera inequidades en el acceso a servicios, especialmente en poblaciones vulnerables. En este contexto, el modelo population health management (PHM) se posiciona como una estrategia para mejorar los resultados en salud mediante la segmentación poblacional, la estratificación de riesgos y la coordinación intersectorial. El presente estudio tuvo como objetivo proponer un modelo conceptual adaptado al sistema de salud peruano para la implementación del PHM en contextos de fragmentación institucional. Se realizó una revisión bibliográfica de alcance de literatura publicada entre 2015 y 2025 en bases de datos internacionales y regionales. Los resultados evidencian brechas en la gestión de enfermedades crónicas, limitada coordinación entre subsistemas y persistentes inequidades en el acceso a servicios de salud. Asimismo, se identificó una escasa producción científica alineada con las prioridades institucionales. Se concluye que la implementación del enfoque PHM en el Perú requiere fortalecer la integración entre subsistemas mediante estrategias de segmentación, estratificación de riesgos y articulación intersectorial, constituyendo el modelo propuesto una herramienta relevante para la gestión y formulación de políticas en salud.
